Keperra Family Practice will be running Covid-19 vaccine clinics commencing 22nd of March 2021 with Phase 1B as per Government schedule.

We follow a strict allocation of vaccines. Please be mindful that appointments can only be made accordingly to the number of vaccines provided by the Government.
If you answer YES to any of the questions or if you have any concerns about being vaccinated, you need:
If you have never attended our surgery please bring the following:
* Form of identification, such as driver's licence, ID card, passport.
* Medicare card.
* A health summary from your regular GP, stating a list of current and past problems, allergies, medications, immunisations.
For all healthcare, Defence, Police, Fire, emergency services and meat processing workers, please ensure you bring proof of occupation along to your appointment.
Please ensure you arrive at the appointment on time and you wear appropriate clothing, such as sleeveless or sleeve pull up shirts.
Please be mindful that if you are not eligible or cannot demonstrate your eligibility when you arrive, we will not be able to vaccinate you.
